Families with an annual income of less than ₹4 lakh are eligible for this scheme. Beneficiaries need to provide proof of income and domicile in Gujarat to qualify.

Application Process For The Mukhyamantri Amrutum Yojana

The application process involves visiting the local Taluka or Civic Center with required documents like income proof, Aadhaar card, and domicile certificate. Upon verification, eligible families receive a smart card to access benefits. Mukhyamantri Amrutam Yojana is said to be offline for which the applicant needs to complete the following steps:-

Step 1: Go to Civic Centre stores or Taluka, civil offices.

Step 2: Provide biometric details such as (Fingerprints; and iris scans).

Step 3: Complete the MA application form and receive the MA health card.

MA Yojana Health Card Details

Recipients have to register at point-of-service kiosks. To get the treatment one is supposed to use this card. The Validity of this MA Yojana Health Card is for 3 years (renewable).

Purpose of the Card: A Health Card is issued to a beneficiary as a document of identity that he or she is registered under MA Yojana. Most of the time it is mandatory to seek treatment through cashless modes at the registered hospitals.
Registration Process: The card can only be obtained by enrolling at the point-of-service kiosks, as part of the requirements for a given beneficiary.
Usage of the Card: The card is utilised for availing necessary medical treatments in empanelled hospitals. It is also scanned at the time the patient comes for treatment to check if the patient is eligible for the coverage.

How To Renew MA Yojana Smart Health Card?

Go to the kiosk or dial the client care centre and submit the Income proof & the previous card. Applicants can renew this MA Yojana Health Card every three years.
